The first step is to go to www.ssa.gov and click on the disability tab. Second, make a list of all the medications you have been prescribed over the years from the point of onset of your disability. The best way is to gather all the prescription bottles you have; it should list the name, date, dose, and prescribing doctor.

Finally, make a list of all your current and previous employers from the past 15 years. This list should include the employers name, address and dates of employment.

Important documents you will need are: Tax return or W2s for last year and a blank check.

Once you have the following information ready to go, you can apply online.
